The Resident Assessment Protocols are 18 problem-oriented guidelines triggered by certain entries on the MDS assessment. RAPs help to determine if a problem exists, and are meant to be a link between the MDS assessment and the care plan. Section V, The RAP Summary sheet, is included in all Admission, Annual, and Significant Change assessments. RAPs checked as triggered in Section V must be addressed in the care-planning process.
This manual defines and explains the Resident Assessment Protocols. Clear instructions and samples are provided for implementing the Triggers, Guidelines, RAP Keys, and RAP Summary documentation. Pertinent excerpts are given from the Revised Long Term Care Resident Assessment Instrument (RAI) User’s Manual, Version 2.0, Centers for Medicare and Medicaid Services, December, 2002, and also from the Revision to the Resident Assessment Instrument Manual, CMS, April, 2004.
The 18 RAP Modules are easy-to-use checklists that summarize the guidelines suggested by the Revised User’s Manual. The applicable MDS sections are listed on the left side of the form, and flow alphabetically in MDS order for fast reference.
