The Revised QI / QM Reports

Excerpts from Revised Facility Guide for the MDS QI / QM Reports, DHHS, September, 2005:

The revised QI/QM reports consolidate QIs and QMs rather than listing them separately.  They include all of the QMs and any QIs not included in the QMs. They are closely affiliated with the Resident Assessment Protocols (RAPs).

A numbering system is used with the domain number followed by a period and then the QI / QM number within the domain.

Domain numbers and domains:

4.    Cognitive Patterns

5.    Elimination / Incontinence

6.    Infection Control

7.    Nutrition / Eating

8.    Pain Management

9.    Physical Functioning

12.  Post-Acute Care Measures

 

Chronic Care Measures

1.1    Fractures, new – incidence

1.2    Falls - prevalence

2.1    Depressed or Anxious, residents who have become more

2.2    Behavior symptoms affecting others – prevalence

2.3    Depression symptoms without antidepressant therapy – prevalence

3.1     Medications, use of 9 or more different

4.1    Cognitive impairment, increased – incidence

5.1    Bowel or bladder incontinence in low-risk residents

5.2    Urinary catheter, indwelling

5.3     Bowel or bladder incontinence, occasional or infrequent, without a toileting plan – prevalence

5.4     Fecal impaction – prevalence

6.1     Urinary tract infection

7.1     Weight loss, 5% or more in last thirty days or 10% or more in last six months

7.2     Tube feeding – prevalence

7.3     Dehydration – prevalence

8.1     Pain, moderate to severe

9.1     Late-loss ADLs, increased dependence

9.2     Chair-bound or bed-bound, most of the time

9.3     Locomotion in room, decreased

9.4     Range of motion, decreased – incidence

10.1   Antipsychotic use, without psychotic or related condition – prevalence

10.2   Antianxiety or hypnotic use – prevalence

10.3   Hypnotic use, more than two times in last week – prevalence

11.1   Restraint. Physical

11.2   Activity, little or no – prevalence

12.1   Pressure ulcers, high-risk residents

12.2   Pressure ulcers, low-risk residents

Facility Characteristics and Facility Quality Measure / Indicator Reports - Used to target specific potential facility problem areas that need investigation

Resident Level Quality Measure / Indicator Report - Used to select appropriate residents for resident samples to address areas of potential concern
