Home The Revised QI / QM Reports Excerpts from Revised Facility Guide for the MDS QI / QM Reports, DHHS, September, 2005: The revised QI/QM reports consolidate QIs and QMs rather than listing them separately. They include all of the QMs and any QIs not included in the QMs. They are closely affiliated with the Resident Assessment Protocols (RAPs). A numbering system is used with the domain number followed by a period and then the QI / QM number within the domain. Domain numbers and domains: 1. Accidents 2. Behavior / Emotional Patterns 3.
